Sometimes situations arise when a seriously ill patient needs high-tech medical care, but does not have the necessary financial resources. It is for this category of citizens that the quota is designed - a document through which, at the expense of state appropriations, the patient is provided surgery.

This document applies only to government agencies.

What operations can you get a quota for - the main types of diseases that give you the right to receive a free quota

All points regarding the provision of quotas to the population for treatment are prescribed in Order of the Russian Ministry of Health of December 29, 2014 (No. 930n).

The list of ailments for which a patient can receive a quota is very long. You can find out more about this list from your doctor, or search online sources - Appendix 4 to the Order of the Ministry of Health and Social Development.

  • Serious disruptions internal organs, which requires their transplantation.
  • Various
  • Carrying out manipulations on open heart.
  • Errors in the functioning of the brain, which can be eliminated through surgery.
  • Congenital pathologies, diseases endocrine system, leukemia.
  • Surgical procedures on the spine.
  • Violation of the functions of the visual organs.

In the above mentioned Order of the Ministry of Health and Social Development there is a column about emergency situations.

For example, if a patient has an extensive myocardial infarction that requires the installation of a stent, but relatives cannot give the clinic guarantees about payment for this very operation, the administration of this medical institution does not have the right to refuse hospitalization.

The patient is required to provide necessary help with further informing the health authority (at the place of registration) about the procedures performed.

Full list therapeutic measures is formalized, and in the future the clinic is paid money for the work performed.

Preparatory examination for obtaining a quota - where to undergo a medical examination?

To obtain the quota in question, patients with a serious illness should visit several medical commissions:

1. Commission at the clinic at the place of registration

Here the relevant specialist prescribes an examination, after which a meeting of the first commission is held. At positive result the patient is given a document signed by the head physician of the clinic stating that the patient needs high-tech treatment. An extract from the medical history is attached to this document.

2.Commission of the regional health department

If the decision is made in favor of the patient, he is provided with a document-coupon with detailed information about the diagnosis and medical history.

3. Commission at the medical institution where they plan to provide treatment

The main points that are clarified during the meeting:

  • Is this institution capable of providing the patient with the necessary medical care?
  • Does the patient have any contraindications to hospitalization?

List of documents for obtaining a quota for a free operation

When planning to begin the procedure upon receiving a quota for surgical treatment, the patient should prepare the following documents:

  • Statement , which contains the patient’s full name, home address, contact phone number, Email(if any), series and number of passport/birth certificate.
  • Written consent to the processing of personal data .
  • Photocopy of passport or birth certificate (if the patient is under 14 years of age).
  • Extract from medical card patient about the history of his illness. This document is issued by the clinic at the place of residence on behalf of the head physician.
  • Results of hardware and laboratory tests , on the basis of which the diagnosis was established. According to regulations Russia, the patient has the right to keep the originals of these documents, and the commission is provided with copies.
  • Photocopy of compulsory health insurance and/or pension insurance policy . This applies to patients who have such evidence. In the absence of policies, the package necessary documents comes without them.

If the patient does not have the opportunity to personally prepare and submit documents, and uses the services legal representative, to the list discussed above are additionally attached:

  1. A copy of the legal representative's passport.
  2. Statement on his behalf.
  3. Official confirmation of representation. It may also be a duly certified power of attorney.

Instructions for obtaining a quota from the Department of Health - where to go and what will be needed?

The procedure in question is as follows:

  1. Visiting your doctor (at the clinic at the place of registration). The doctor decides what tests need to be taken and writes out a referral for diagnostics.
  2. Execution of an extract from the medical history by the doctor mentioned above. The results of the survey are also included here. The specified document must be certified by the signature of the head physician and the seal of the medical institution.
  3. Sending the prepared set of documentation to the local health department. This is done by the clinic staff upon completion of the medical examination. It would not hurt the patient to clarify and remember the date of dispatch. If a negative decision is made, the applicant is given a protocol explaining the reasons for the refusal.
  4. The Department of Health makes a decision on issuing a quota. In some cases, this procedure requires the patient’s personal presence, but often they do without it. This all takes 10 working days: after this period, the applicant must contact his clinic and find out about the answer. In emergency situations, when surgical intervention must be performed urgently, the attending physician attaches a corresponding note to the package of documents: this speeds up the process of obtaining a quota.
  5. Sending documents to a specialized clinic. Often the patient has to wait in line for hospitalization. The date of appearance at the medical institution can be found on the website talon.gasurf.ru. Upon hospitalization, the patient must present originals medical documents.

Instructions for obtaining a quota in the clinic through the quota committee

This method of obtaining a quota is more preferable: the patient can personally choose a medical center for treatment, and the procedure itself lasts, on average, 2 weeks.

The algorithm of actions in this situation is as follows:

  1. After receiving the necessary package of documents from the clinic at the place of registration with a positive decision of the commission, the patient independently looks for an institution where he will be provided high-tech treatment.
  2. Specialists from the specified medical institution consider the patient’s request, convene a “quota committee”, which considers the possibility of hospitalization under a quota.
  3. An officially issued decision, along with a list of other documents, is sent to the local health department.

Queue for quota surgery – what do you need to remember?

Those who are planning to receive a quota for surgical treatment need to remember the following points:

  • You can find out how the queue is progressing on special websites (one of them has already been mentioned above) or in the clinic where assistance will be provided.
  • You can find out about the availability of quotas from your local health department or medical facility. , where high-tech treatment will be carried out. Each such institution has its own quota department, where specialists will tell you whether there are still coupons for free operations and how many are left.
  • If surgery is needed urgently , and the quotas have ended, the patient can pay all expenses, and in the future - submit a set of documents to the health department. In this way, you can offset the costs of treatment.
  • In difficult situations, when surgery is urgently needed and there is a quota, the clinic has no right to refuse help. In other cases, patients will wait their turn: the legislation does not provide for any benefits to help speed up the provision of treatment.

Obtaining a quota for high-tech medical care

Before release Federal Law « On the basics of protecting the health of citizens in Russian Federation » the procedure for referring patients to high-tech medical care (issuing quotas for high-tech medical care) was approved every year by the Ministry of Health and social development. Released in 2012 new order, which will be valid indefinitely.

Last year, in order to receive a quota for VMP, a citizen had to apply directly to the regional health authority (department, committee, ministry), having in hand an extract from medical documentation, which includes recommendations for obtaining VMP, and a number of other documents (copies of passport, compulsory medical insurance policy and pension certificate). The structure of the health authority provided for the corresponding commission of the constituent entity of the Russian Federation for the selection of patients for the provision of medical care, which made a decision within 10 working days from the date of application.

Now, in accordance with the procedure approved by Order of the Ministry of Health and Social Development of December 28, 2011 No. 1689n, the procedure has partially changed.

Quotas for high-tech medical care

Let's take a closer look at the procedure for obtaining a quota.

The decision to grant a quota is made by the Commission of the health authority of the constituent entity of the Russian Federation for the selection of patients for the provision of VMT.

However, now the selection of patients and their referral to this commission is carried out by the medical commissions of those medical organizations in which patients are treated and observed, on the recommendation of the attending physician based on an extract from the patient’s medical records.

An extract from the patient's medical records, drawn up by the attending physician, must contain a diagnosis of the disease (condition), a diagnosis code according to the ICD, information about the patient's health status, diagnosis and treatment, and recommendations on the need for VMP. It is accompanied by the results of laboratory, instrumental and other types of studies on the profile of the patient’s disease, confirming the established diagnosis.

The medical commission, within three working days from the date of receipt of the extract, reviews it and makes a decision on sending or refusing to send the patient’s documents to the Commission of the constituent entity of the Russian Federation to resolve the issue of the need to provide him with VMP. The decision of the Medical Commission is documented in a protocol.

The criterion for making a decision by the medical commission is the presence of medical indications for the provision of VMP in accordance with the list of types of VMP.

If the medical commission makes a decision to send the patient’s documents to the Commission of a constituent entity of the Russian Federation, within three working days it generates and sends a set of documents to the health authorities. In this case, the patient has the right to receive a protocol of the decision of the medical commission and an extract from the medical documentation in hand in order to independently take the documents to the health authorities.

If the medical commission decides to refuse, then the patient is given a protocol of the decision indicating the reasons for the refusal and an extract from the medical documentation.

The set of documents sent to the Commission of a constituent entity of the Russian Federation must contain:

  1. An extract from the protocol of the decision of the Medical Commission.
  2. A written statement from the patient (his legal representative, proxy), containing the following information about the patient:
  1. information about place of residence,
  • Consent to the processing of personal data of a citizen (patient).
  • Copies of the following documents:
    1. passport of a citizen of the Russian Federation,
    2. patient's birth certificate (for children under 14 years of age),
    3. compulsory medical insurance policy for the patient (if available),
    4. certificate of compulsory pension insurance of the patient (if available),
    5. an extract from the patient’s medical records signed by the head of the medical organization at the place of treatment and observation of the patient,
    6. results of laboratory, instrumental and other types of studies confirming the established diagnosis.

    In the case of an application on behalf of the patient by the patient’s legal representative (authorized person):

    1. The written application additionally indicates information about the legal representative (authorized representative):
    1. last name, first name, patronymic (if available),
    2. information about place of residence,
    3. details of the document proving identity and citizenship,
    4. postal address for sending written responses and notifications,
    5. contact phone number (if available),
    6. email address (if available).
  • In addition to the patient's written request, the following must be attached:
    1. a copy of the passport of the patient’s legal representative (patient’s proxy),
    2. a copy of a document confirming the powers of the patient’s legal representative, or a power of attorney certified in accordance with the procedure established by the legislation of the Russian Federation in the name of the patient’s authorized representative.

    The commission of a constituent entity of the Russian Federation resolves the issue no later than 10 working days from the date of receipt of the documents. Based on the results of reviewing the documents, one of the following decisions is made:

    1. On the availability of medical indications for referring the patient to a medical organization for the provision of VMP. In this case, the chairman of the commission ensures that the patient is issued a coupon for the provision of VMP (form N 025/u-VMP) within 3 working days, as well as agrees on the expected date of hospitalization with the medical organization for which the quota is requested. After this, the patient is referred for treatment.
    2. About the absence of medical indications for referring the patient to a medical organization for the provision of VMP.
    3. On the availability of medical indications for referring the patient to a medical organization for the purpose of additional examination. In this case, the health authority ensures that the patient is referred for the necessary examinations.
    4. On the availability of medical indications for referring the patient to a medical organization for the provision of specialized medical care. In this case, the health authority also ensures that the patient is referred for treatment to the appropriate medical organization.

    In addition to local health authorities, a quota can be obtained by submitting a written application to the Ministry of Health and Social Development of Russia, but this is allowed only in cases where:

    1. the patient does not live on the territory of the Russian Federation;
    2. the patient is not registered at his place of residence;
    3. The health authority of a constituent entity of the Russian Federation did not ensure the patient was referred to medical organizations to provide VMP.

    Hospitalization is carried out by decision of the commission of the medical organization providing medical treatment. This decision is made no later than 10 working days on the basis of a coupon for the provision of medical care with electronic attachment of documents from the above list.

    How to get a VMP quota?

    Discovering that you have a disease that requires high-tech surgical intervention is a common story. Such an intervention can be performed using minimally invasive surgery using the da Vinci robotic platform. The basis of the method is surgery using pinpoint punctures in tissues or through natural physiological openings, which avoids large postoperative marks.

    It may seem to you that it is extremely difficult to obtain funding from the budget for high-tech assistance, however, there is this chance, and the number of operations performed under the quota is growing every year. According to official data, expenses for VMP grow by 20% annually, and the quantitative indicator of operations performed over the past 10 years has increased 15 times.

    Speaking at SPIEF 2018, Minister of Health Veronika Skvortsova reported on the increased availability of high-tech medical care (HTMC): “We have significantly increased the volume of high-tech medical care, starting with 60 patients 10 years ago, and now it is more than 1 million according to last year’s results,” - said the Minister

    An operation using the da Vinci robotic platform is expensive, but any patient in need has the right to receive a state quota for such an operation.

    We bring to your attention answers to questions that applicants have.

    Who is eligible for free high-tech surgery?

    Any citizen of the Russian Federation in need can benefit from free medical care. This is stated in the Decree of the Government of the Russian Federation dated December 8, 2017 N 1492 “On the Program of State Guarantees for the provision of free medical care to citizens for 2018 and for the planning period of 2019 and 2020.” This document is adopted annually.

    Document

    Order of the Ministry of Health and Social Development No. 1248n dated December 31, 2010 regulates the procedure for providing High-Tech Medical Care (HMC) to citizens of the Russian Federation at public expense. According to this order, every ordinary citizen of the Russian Federation has the right to receive funding for an operation from the state if necessary.

    Which institutions deal with the issue of quotas?

    All issues of obtaining funding for High-Tech Medical Care (HTMC) from the federal budget are regulated by the Ministry of Health.

    What diseases are covered by the quota program?

    The list of diseases for which a patient can count on state support is approved annually by order of the Ministry of Health of the Russian Federation.

    The types of assistance that can be provided using the da Vinci robot can be viewed here.

    Which institutions have the right to provide High-Tech Medical Care (HMC)?

    A medical institution providing High-Tech Medical Care under state quotas must have an appropriate license. All clinics on our list that have the da Vinci robotic system in their arsenal have such a document.

    Are there any age restrictions for the patient?

    There is no age limit for minimally invasive surgery using the da Vinci robot.

    What steps must be taken in the process of obtaining quota for gold standard surgery?

    Step 1. Contact your doctor.

    First of all, the patient should contact the attending physician to obtain a referral for hospitalization, registration necessary documents and forwarding them for consideration to the competent organization. The attending physician of the medical organization in which the patient is being diagnosed and treated determines the presence of medical indications for the provision of VMP and, if there are medical indications, issues a referral for hospitalization. The presence of medical indications is confirmed by the decision of the medical commission of the medical organization, which is documented in a protocol and entered into the patient’s medical documentation. If there are medical indications, the attending physician issues a referral for hospitalization.

    Requirements for making a referral for hospitalization:

    The referral must be completed on the letterhead of the referring medical organization legibly by hand or in printed form, certified by the personal signatures of the attending physician and the head of the medical organization, as well as the seals of the attending physician and medical organization, and contain the following information:

    • compulsory medical insurance policy number and name of the medical insurance organization;
    • insurance certificate of compulsory pension insurance;
    • diagnosis code of the underlying disease in accordance with the international classification of diseases;
    • profile and name of the type of VMP;
    • name of the medical organization to which the patient is sent;
    • FULL NAME. and the position of the attending physician, if available - his telephone number and email address.

    Attached to the direction:

    • an extract from the medical documentation indicating the diagnosis of the disease, the disease code according to the international classification of diseases, information about the state of health, the results of special medical research. The extract must be certified by the personal signatures of the attending physician and the head of the medical organization;
    • a copy of the patient’s identification document or a copy of the birth certificate (for children under 14 years of age);
    • a copy of the compulsory medical insurance policy;
    • a copy of the insurance certificate of compulsory pension insurance (if available);
    • consent to the processing of personal data.

    The head of the referring medical organization or another employee of the medical organization authorized by the head transfers a referral for hospitalization:

    — to the receiving medical organization, if VMP is included in the basic compulsory medical insurance program (clause 15.1 of the Procedure);

    - to the executive body of the constituent entity of the Russian Federation in the field of healthcare (OHC), if VMP is not included in the basic compulsory medical insurance program.

    Important: The patient or his legal representative has the right to submit a completed package of documents independently. This will speed up the collection and provision of the documentation necessary to obtain VMP.

    Step 2. You must wait for the VMP coupon to be issued.

    There are 2 options for issuing a ticket:

    • If the patient is referred to the provision of medical care included in the basic compulsory medical insurance program, then the issuance of a coupon with a set of documents attached, specified in step 1, is provided by the receiving medical organization.
    • If the patient is referred to the provision of primary care that is not included in the basic compulsory medical insurance program, the registration of a coupon and the conclusion of the commission of the executive body of the constituent entity of the Russian Federation in the field of healthcare for the selection of patients for the provision of primary care (the OHC commission) is provided by the OHC.

    The OHC commission makes a decision on the presence or absence of indications for referring the patient to the receiving medical organization within 10 working days from the date of receipt of the full package of documents.

    How to get a quota for surgery and treatment in 2018

    The decision of the OHC commission is formalized in a protocol, which must contain a conclusion on the indications for referral to VMP or on the need for additional examination.

    Note: An extract from the protocol of the decision of the OHC commission is sent to the referring medical organization, and is also handed over to the patient (his legal representative) upon written application or sent to the patient (his legal representative) via postal and (or) electronic communication.

    Step 3.It is necessary to wait for the decision of the commission of the medical organization providing VMP.

    The commission makes a decision on the presence (absence) of medical indications or the presence of medical contraindications for hospitalization of the patient within seven working days from the date of issuing a coupon for the provision of medical treatment.

    The decision is formalized in a protocol containing a conclusion on the presence of medical indications and the planned date of hospitalization of the patient, on the absence of medical indications for hospitalization, on the need for additional examination, on the presence of medical indications for referring the patient to a medical organization for the provision of specialized medical care, on the presence of medical contraindications for hospitalization of the patient to a medical organization providing medical care.

    Step 4. Upon completion of VMP, receive recommendations.

    Based on the results of providing VMP, medical organizations give recommendations on further observation and (or) treatment and medical rehabilitation with the preparation of appropriate entries in the patient’s medical records.

    Note: In case of dissatisfaction with the quality of medical care, the patient has the right to contact local authorities health departments or territorial bodies of Roszdravnadzor.

    What should applicants for the da Vinci robotic surgery quota take into account?

    The number of quotas is disproportionately less than the number of patients in need of surgery. The classic direct path to obtaining quotas usually takes time.

    Where can I find out if there are quotas for High-Tech Medical Care?

    The Ministry of Health annually approves the number of quotas for VMP and other types of treatment. All quotas are distributed among medical institutions licensed to provide such care. Information about how many quotas remain can be found from two sources. One of them is the Department of Health, the other is the clinic where you want to get VMP.

    In any clinic that provides treatment under state quotas, there is always a person responsible for quotas, or there may even be an entire quota department. This is where you should contact regarding quota availability.

    How many high-tech operations using the da Vinci robot are performed per year, is there a chance to get help?

    In 2017, a total of 2,421 operations were performed using a robotic system. Of these, only 5% was paid for by private individuals; the rest were financed through quotas.

    If a medical center is equipped with a da Vinci system, does this mean that medical center is it possible to get all types of operations?

    The use of a robotic system makes it possible to perform complex interventions in urology, general surgery, gynecology, thoracic surgery, colorectal surgery and on the head and neck organs. Although the list of operations is quite wide, 70% of all interventions are performed in Urology, and robot-assisted prostatectomy is the gold standard in the world for the treatment of prostate cancer. It is important to understand that each clinic develops separate areas. There are multidisciplinary centers where surgical interventions using da Vinci are carried out in different areas, and there are centers specializing in one thing. For example, GBUZ MO "MONIIAG" specializes in gynecology, and all operations are performed only in this area.

    Which medical institutions in Russia provide VMP using the da Vinci robot?

    A list of medical institutions that perform surgical interventions using the da Vinci robot is presented on our website, in the “Clinics” section. There you can find out the address and telephone number of the reception of each medical center.

    First of all, the patient should contact the attending physician to receive a referral for hospitalization, prepare the necessary documents and send them for review to a competent organization.

    • FULL NAME. the patient, his date of birth, registration address;
    • compulsory medical insurance policy number and name of the medical insurance organization;
    • insurance certificate of compulsory pension insurance;
    • diagnosis code of the underlying disease in accordance with the international classification of diseases;
    • profile and name of the type of VMP;
    • name of the medical organization to which the patient is sent;
    • FULL NAME. and the position of the attending physician, if available - his telephone number and email address.

    Attached to the direction:

    • an extract from the medical documentation indicating the diagnosis of the disease, the disease code according to the international classification of diseases, information about the state of health, the results of special medical studies. The extract must be certified by the personal signatures of the attending physician and the head of the medical organization;
    • a copy of the patient’s identification document or a copy of the birth certificate (for children under 14 years of age);
    • a copy of the compulsory medical insurance policy;
    • a copy of the insurance certificate of compulsory pension insurance (if available);
    • consent to the processing of personal data.

    The head of the referring medical organization or another employee of the medical organization authorized by the head transfers a referral for hospitalization:

    — to the receiving medical organization, if VMP is included in the basic compulsory medical insurance program (clause 15.1 of the Procedure);

    Step 2.

    Protection from extradition

    You need to wait for the VMP coupon to be issued.

    Based on the results of the provision of medical treatment, medical organizations provide recommendations for further observation and (or) treatment and medical rehabilitation with the preparation of appropriate entries in the patient’s medical records.

    Note: In case of dissatisfaction with the quality of medical care, the patient has the right to contact local health authorities or territorial bodies of Roszdravnadzor.

    If a medical center is equipped with the da Vinci system, does that mean that all types of surgeries can be obtained at the medical center?

    List of operations that are performed using the da Vinci robot-assisted system in Russia:

    Head and neck: Glossectomy, Thymectomy, Theriodectomy, Hemithyroidectomy, Resection of the thyroid isthmus.

    Hello! Please help me with advice -
    My mother, 70 years old, went to the doctor 4 months ago with pain and lumps in her chest, and says that almost immediately she was put on the waiting list for surgery to remove the tumor. Only a few days ago, after all the tests, she was confirmed that it was breast cancer, C50E, and was told that the operation needed to be done within a month. At the same time, the turn for the operation has not yet arrived and no one knows when it will be (??). And of course they offered some kind of paid scheme for an expedited operation at some unimaginable price (I’m not sure if this is even legal...).

    What documents do you have on hand (apparently, the necessary set for the operation): Clinical analysis blood, ecg, biochemistry, rw hiv hepatitis bc, general analysis blood, x-ray of the lungs, mammography, ultrasound of the mammary glands, abdominal cavity, pelvis, conclusion of a therapist and gynecologist and a referral from an oncologist. All that remains is to do a biopsy. I don’t understand why they didn’t target her earlier. And I also understand why they didn’t say what stage of cancer, maybe it can’t be determined without a biopsy?

    Where was examined: Hospital No. 57 in Moscow, Mikhail Bannikov

    Questions:
    - Tell me, please, is it abnormal to have such queues in Moscow for urgent surgery? Mom didn’t understand, but maybe she’s waiting for her turn for the quota? But at the same time, I read that in order to register in the queue for quotas, the patient himself must contact the health department, but she did not do this. Or could the hospital send the documents themselves? Or are there no quotas needed for breast removal surgery?
    - Do I understand correctly that it makes sense to contact another institution (62nd, Herzen, Blokhin)? They will ask you to carry out all the examinations again, and will they not accept documents from Hospital No. 57? Could it turn out that the operation will be scheduled there faster?
    - Probably, we urgently need to undergo a paid emergency operation? In one of the above hospitals? Please tell me what is the normal price for such an operation? Somewhere they write that the money for it can then be returned through the Ministry of Health, but this is probably a fantasy.
    - Is it normal that the doctors didn’t say what stage of the cancer, maybe it can’t be determined without a biopsy?

    Thank you very much in advance for your advice and understanding!

    Julia123, hello.
    Waiting for the patient to receive special medical care for cancer within 4 months. not only abnormal, but also illegal!

    Decree of the Government of the Russian Federation dated November 28, 2014 N1273 approved. The program of state guarantees of free medical care to citizens for 2015 and for the planning period of 2016 and 2017, in which the waiting period for routine medical care in a hospital is set at 30 days.

    Decree of the Moscow Government dated December 23, 2014 N 811-PP approved. Territorial program of state guarantees of free medical care to citizens in the city of Moscow for 2015 and the planning period of 2016 and 2017.
    Clause 2.8 of the Appendix provides that medical care in inpatient conditions as planned ( planned hospitalization) is no later than 14 working days from the date the attending physician issues a referral for hospitalization of the patient.

    The waiting time for high-tech medical care (according to the quota) may be different, but from your message it follows that we are talking about providing high-tech assistance does not work, quota registration was not offered.

    Who diagnosed your mother and what? Who prescribed surgical treatment? Did she receive a referral to a medical institution to provide her with specialized medical care in a hospital setting? Did you go to the hospital after that? There are doubts about whether the patient would be put on a waiting list for surgery immediately after going to a medical facility with complaints and before a diagnosis is made.

    If in a medical institution where the patient is sent for specialized inpatient care, such assistance cannot be provided within the established time frame, then the patient must be referred to an institution where such assistance can be provided.

    I'll make a few more additions later.

    Best regards, Love.

    PS: Oncological diagnosis requires morphological confirmation (histology, IHC). The final stage of the disease is determined by the results of histological examination of the surgical material, taking into account the prevalence of the oncological process.

    Imposing paid services for the provision of medical care, which should be provided free of charge (included in the list of state guarantees), under the pretext that the wait free help can be at least 4 months, - money scam. And who told your mother about this waiting period?

    At the Herzen Institute, surgical treatment is carried out on a paid basis and according to a quota. From the experience of my relative (non-resident): 3 years ago I came in with the results of examinations and a diagnosis of breast cancer, the examination was carried out again in the conditions of the institute, they trust only their specialists. The examination cost about 100 thousand rubles. The operation was based on a quota; there were no problems with registration. After the operation, the patient was told the desired amount of “gratitude” (similar to “tradition”), the patient is in the hospital, postoperative period, waiting for histology results, relatives are looking for opportunities.

    It is possible to talk about the judicial prospect of recovering the costs of treatment, which is included in the list of guaranteed free treatment for a citizen, only if indisputable evidence is presented to the court that the patient applied for such help, the guaranteed care was not provided free of charge, including within the stipulated time frame, and the patient was forced to apply for medical care on a paid basis. There are also some nuances with the processing of such payment.

    What to do: do a biopsy and get histology results from the biopsy, at the same time figure out whether the mother received a referral to a medical institution for specialized inpatient medical care (surgical treatment), whether she applied to this medical institution and is included in the waiting list. If not, this must be done. If at the same time she is informed about long term waiting for medical assistance, you should contact the head doctor of the medical institution or his deputy at therapeutic work for an appointment and submit a written request to ensure the provision of medical care within the prescribed time frame (have a copy with a mark of acceptance on hand). The content of the appeal must be to the point. If necessary, let me know and I’ll sketch out a sample. This will probably be enough. If not, then contact the Moscow Department of Health (there is an opportunity to contact them on the website) with questions about the legality of the “proposals” about paid services, and with a request to ensure the provision of medical care in a timely manner.
