pharmachologic effect

Myotropic antispasmodic, isoquinoline derivative. Inhibits phosphodiesterase (PDE) IV, which leads to the accumulation of intracellular cyclic adenosine monophosphate (cAMP) and, as a result, inactivation of the light chain of myosin kinase, resulting in relaxation of smooth muscles.

Regardless of the type of autonomic innervation, drotaverine acts on smooth muscles in the gastrointestinal tract, biliary, genitourinary and vascular systems Oh. In the myocardium and blood vessels, the enzyme that hydrolyzes cAMP is PDE III, which explains the absence of serious side effects from the outside of cardio-vascular system(SSS) and unexpressed therapeutic effect in relation to the SSS.

The presence of a direct effect on smooth muscles allows its use as an antispasmodic in cases where drugs from the group of m-anticholinergics are contraindicated (angle-closure glaucoma, hyperplasia prostate gland).

Pharmacokinetics

When taken orally, absorption is high, half-life is 12 minutes. Bioavailability – 100%. Evenly distributed in tissues, penetrates smooth muscle cells. The time to reach Cmax in the blood is 2 hours. Protein binding is 95-98%. It is mainly excreted by the kidneys, and to a lesser extent – with bile. Does not penetrate the blood-brain barrier.

Indications

- spasm of smooth muscles of the urinary and biliary organs ( renal colic, pyelitis, tenesmus, biliary colic, intestinal colic, dyskinesia of the biliary tract and gallbladder of the hyperkinetic type, cholecystitis, postcholecystectomy syndrome);

- spasm of smooth muscles of the gastrointestinal tract (as a rule, as part of combination therapy): pylorospasm, gastroduodenitis, peptic ulcer stomach and duodenum, spastic constipation, spastic colitis, proctitis;

- tensor headache;

- when carrying out some instrumental studies, cholecystography.

Contraindications

- pronounced hepatic and renal failure;

- severe deficiency (low cardiac output);

- lactation period;

increased sensitivity to the components of the drug.

This dosage form is not used in children under 3 years of age.

Due to the presence of lactose monohydrate (milk sugar) in the drug, its use is contraindicated in cases of congenital lactose intolerance, lactase deficiency, and glucose-galactose malabsorption.

Drotaverine should be taken with caution used for arterial hypotension, severe atherosclerosis coronary arteries, prostatic hyperplasia, angle-closure glaucoma, during pregnancy.

No-spa as a substitute for Drotaverine is quite suitable in its qualities and is sometimes even more effective. But that's only for certain people, and Drotaverine has even more advantages both in price and in the possibility of use.

What does drotaverine, an effective and often prescribed drug with an antispasmodic effect, help with? It has a beneficial effect on smooth muscle tissue various systems body, eliminating spasms and painful sensations both nervous and muscular etiology.

No serious side effects were noted, but the use medicine should be carried out exclusively by a doctor and taken by the patient under the supervision of a specialist.

Drotaverine: composition and pharmacological action

The drug is available in two dosage forms is a solution for intramuscular injections and tablets. The first includes:

  • main component – drotaverine hydrochloride;
  • additional substances.
  • drotaverine hydrochloride;
  • lactose;
  • additional components.

The drug is prescribed in tablet form more often than in injections.

After taking the drug, the active substance, which has antispasmodic activity, begins to affect the smooth muscles that are located in the genitourinary, biliary, cardiovascular and gastrointestinal systems. It significantly dilates the blood vessels, as a result of which oxygen begins to flow freely to the tissues and relieves spastic pain.

The components of the drug are capable of changing the permeability and potential of membranes in the tissue in a short period of time. They also reduce the activity of phosphodiesterase, a special enzyme.

After taking the tablet the concentration active substance will reach its concentration in 50-60 minutes. Excreted from the body in urine, a small amount may be excreted in feces.

At the same time, the drug Drotaverine does not have any serious side effects in relation to the cardiovascular system, comparing favorably in this regard with many of its analogues. It is a drug with antispasmodic, vasodilating, myotropic and hypotensive effects.

Indications for use

Why Drotaverine can be prescribed by a therapist - the specialist must exclude individual intolerance to the components of the drug. The main indication for its use is to relieve a person of spastic pain that can occur with a variety of ailments. The drug may be prescribed if one of the following diseases has been diagnosed:

  • frequent spasms with cholecystitis;
  • stomach or duodenal ulcer;
  • constipation, accompanied by spasms of smooth tissues that cause severe pain;
  • colic due to gas retention;
  • urethritis and cystitis;
  • proctitis with pronounced pain;
  • urethrolithiasis, pyelitis, nephrolithiasis - for these diseases, Drotaverine is prescribed as a local anesthetic drug, and it is part of a complex of medications.

Along with this, a medicine such as Drotaverine can be recommended for use when diagnostic activities. For example, with cholecystography or similar.

Elimination of spasms is the main task of Drotaverine

Because the drug has a vasodilating effect, health care providers often prescribe it to relieve severe headaches that occur when cerebral vasospasms occur.

The use of Drotaverine in gynecology is widespread - it can help with spasms of the uterine pharynx or with prolonged dilatation. It is also prescribed for emergency childbirth if the specialist notes the risk of uterine spasms during the passage of the fetus.

Taking Drotaverine

The tablets are taken orally and should be taken with plenty of water. The frequency of meals, as a rule, does not have any effect, so there is not much difference in taking Drotaverine before or after meals. You can even do it on an empty stomach if a person suffers from severe pain or colic. The only inconvenience in this case is that the impact on the source of discomfort may begin a little later.

If the patient suffers too much strong pain, which you have no strength to endure, the doctor may prescribe injections. They act much faster because they are administered intramuscularly. However, there are exceptions for patients with hepatic or renal colic - in them the drug is administered intravenously (slowly and in small portions). At the same time, there is a small risk of collapse (decreased blood supply to tissues, resulting in dysfunction of certain organs), so such procedures should be carried out exclusively in a supine state.

Important: it is prohibited to independently determine the duration of the course; only a specialist can choose how to take Drotaverine.

In particular in case of emergency When vasospasm occurs and peripheral circulation is disrupted, Drotaverine can even be injected into an artery. The dosage must be carefully calculated by the doctor, and the drug should be administered even more slowly than intravenously.

Important: since the procedure is emergency, it should only be performed by a specialist. He will be able to stop possible consequences(although, as a rule, with proper actions medical worker they do not appear).

Compatibility of the drug with other drugs

If you use Drotaverine for the abdomen or in other cases in combination with other drugs that also have an antispasmodic effect, the effect of treatment may be enhanced, but such therapy should be carried out exclusively with the approval of the attending physician. Only a specialist will recommend medications that not only do not cancel therapeutic effect Drotaverine, but can also make it more pronounced.

If you combine Drotaverine with tricyclic antidepressants, you should be prepared for the fact that arterial hypotension may increase many times over.

Patients who have been prescribed morphine should use Drotaverine with caution, as this drug greatly reduces the spasmogenic effect.

Overdose of Drotaverine

It is strictly forbidden to change the dosage prescribed by your doctor. An increase in the number of tablets or injections can provoke the development of atrioventricular block and lead to paralysis of the respiratory nerve, and the consequences of this can be extremely negative.

Possible side effects

Side effects from taking Drotaverine can occur in two cases: if it is taken for too long or if allergic reaction to any component. Such actions include the following manifestations:

  • gastrointestinal tract - belching, constipation (or vice versa diarrhea), feeling of nausea, vomiting;
  • central nervous system – fainting, severe headaches, disrupted daily routine, sleep disturbances;
  • respiratory tract – swelling of the mucous membranes or bronchospasms;
  • allergic reactions - Quincke's edema, dermatitis, rash, excessive sweating, itching;

All the described effects occur both when taking tablets and after injections, however, in the latter case, the patient may begin to develop depressed breathing, hypotension, or atrioventricular block (impaired conduction of electrical impulses from the atria to the ventricles).

It is important to follow the dosage of the drug, otherwise side effects may occur.

If you have headaches while taking Drotaverine or one of these side effects has been noted, and especially after injections, then the course of treatment should be stopped and you should consult your doctor. He will be able to either select analogues or prescribe additional medications to eliminate these actions.

Use during pregnancy and lactation

Drotaverine hydrochloride, the active substance, is able to penetrate the hematoplacental barrier. In other words, this component of the drug can have a direct effect on the fetus and in one way or another influence its development. A specialist may prescribe medication if the hypothetical benefit for the pregnant woman is higher than the potential threat to the child.

During breastfeeding You should stop taking Drotaverine, since it passes into mother's milk and, accordingly, is passed on to the baby. If refusal is impossible, then you need to think about temporarily stopping breastfeeding and transferring the child to artificial formula.

Often, medicine can be prescribed during premature birth, to relieve the tone of the uterus (this will stabilize the condition if there is a threat of miscarriage and will help promptly and without harm to the mother’s health to begin all the necessary auxiliary actions), as well as in the period after childbirth to stop contractions.

Whether Drotaverine can be prescribed to children is an open question. Some pediatricians and therapists are against therapy using this medicine and strongly recommend using it only after reaching 12 years of age, while others believe that the drug will not cause harm and prescribe it from 5-6 years of age. In any case, the treatment process with this medicine should be under the constant supervision of the attending physician.

Contraindications for use

As a rule, almost all medicines have certain contraindications, and Drotaverine is no exception. It should be used only after reading the instructions and according to the prescribed course of treatment. Contraindications are identical regardless of which form of the drug was chosen. These include:

  • heart failure;
  • cardiogenic shock;
  • low blood pressure;
  • atrioventricular block II or III degree;
  • renal or liver failure;
  • congenital lactase deficiency;
  • malabsorption.

The drug is prescribed with caution to those diagnosed with atherosclerosis of the coronary arteries, the same applies to the usual form of atherosclerosis. It is not recommended for people whose professional activity associated with increased concentration. If, nevertheless, taking Drotaverine is necessary, then after it you should avoid activities that require increased attention and concentration for 2-3 hours.

Pharmacological action of drotaverine
Drotaverine is an isoquinoline derivative that exhibits an antispasmodic effect on smooth muscle cells by inhibiting the enzyme phosphodiesterase IV (PDE IV). The consequence of this is an increase in the concentration of cAMP, which inactivates the light chain of myosin kinase, which, in turn, leads to relaxation of smooth muscles.

Drotaverine is effective for spasms of smooth muscles of both nervous and muscular etiology. Regardless of the type of autonomic innervation, drotaverine acts on the smooth muscle tissue cells of the walls of the hollow organs of the digestive, genitourinary and cardiovascular systems.

Professional medical articles regarding the use of drotaverine in the treatment of the gastrointestinal tract
  • Belousova E.A. Antispasmodics in gastroenterology: comparative characteristics and indications for use // Pharmateka. - 2002. - No. 9. – p. 40–46.
On the website in the “Literature” section there is a subsection “Anspasmodics” containing publications for healthcare professionals addressing the use of antispasmodics in the treatment of diseases of the gastrointestinal tract.
Indications for use of drotaverine

Drotaverine is close in chemical structure and mechanism of action to papaverine. Both are phosphodiesterase (PDE) type IV inhibitors and calmodulin antagonists. At the same time, the selectivity of the action of drotaverine in relation to PDE is noticeably higher and the selectivity of its effect on smooth muscles is 5 times higher than that of papaverine.

Drotaverine is effective for spasms of smooth muscles of both nervous and muscular etiology. Regardless of the type of autonomic innervation, drotaverine affects the smooth muscles of the digestive, urogenital and vascular systems.

In the treatment of pain of mild and moderate intensity in the abdomen and pelvis, drotaverine, along with other antispasmodics (papaverine, mebeverine, hyoscine butyl bromide, otilonium bromide and others) is a first-stage drug, which, in the absence of a positive effect with monotherapy with antispasmodics and with long-term and for increasing abdominal pain, they are replaced with second-stage drugs.

Drotaverine has a more pronounced antispasmodic effect than papaverine and usually quite effectively relieves acute spasms of various origins. However, when chronic pathology, such as irritable bowel syndrome or biliary tract disorders, oral administration of these drugs in therapeutic doses is often insufficient, and it becomes necessary to increase their dose or parenteral administration. Although drotaverine and papaverine are generally well tolerated, in large doses or with intravenous use they can cause dizziness, decreased myocardial excitability, impaired intraventricular conduction, up to the development of atrioventricular block. In such a situation, drotaverine and papaverine should be replaced by mebeverine, which has selectivity against the smooth muscles of the digestive tract, mainly the colon, and does not affect the smooth muscle walls of blood vessels.

There are a number of publications in which prominent Russian gastroenterologists express the opinion that in the treatment of gastrointestinal diseases, mebeverine is more effective and has fewer side effects than drotaverine.

Also, leading Russian gastroenterologists warn that drotaverine, like other myogenic antispasmodics, due to their relaxing effect on the lower esophageal sphincter, are not recommended for gastroesophageal reflux disease.

Contraindications:

  • hypersensitivity to drotaverine
  • severe liver or kidney failure
  • severe heart failure (low cardiac output syndrome)
  • Children under 1 year of age are not recommended to take Drotaverine tablets.
In case of hypotension, the use of drotaverine requires increased caution.
Doses and methods of use of drotaverine
  • adults should take drotaverine 2-3 times a day so that the daily dose is 120-240 mg
  • children over 6 years old take drotaverine 2–5 times a day based on a daily dose of 80–200 mg
  • children from one to 6 years old take drotaverine 2-3 times a day so that the daily dose is 40-120 mg
Side effects when taking drotaverine
Rarely - nausea, constipation, headache, dizziness, insomnia, rapid heartbeat. Very rarely - hypotension.

Caution is required when joint use drotaverine with levodopa, since drotaverine reduces the antiparkinsonian effect of the latter and may increase tremor and rigidity.

Due to the fact that drotaverine tablets contain lactose, patients suffering from lactose intolerance may have problems with gastrointestinal tract. Therefore, drotaverine tablets should not be prescribed to patients suffering from lactose deficiency, galactosemia, or impaired absorption of glucose or galactose.

Due to the lack of necessary clinical data during pregnancy prescribing drotaverine is possible only with a careful assessment of the benefit-risk ratio, and when breastfeeding, it is not recommended to prescribe drotaverine .
