A number of dangerous diseases early stage is asymptomatic. If diagnosed early, many of these diseases can be cured.

If you are over 21 years old and your age is divisible by three, contact your clinic and undergo a free examination.

Main goals of medical examination:

Early detection of chronic non-communicable diseases, which are the main cause of disability and premature mortality of the population Russian Federation(hereinafter referred to as chronic non-communicable diseases), which include:

- diseases of the circulatory system and, first of all, ischemic disease heart and cerebrovascular diseases;
malignant neoplasms;
diabetes;
chronic diseases lungs.

These diseases cause more than 75% of all mortality in our country. In addition, medical examination is aimed at identifying and correcting the main risk factors for the development of these diseases, which include:

- increased blood pressure;
- increased cholesterol levels in the blood;
- increased blood glucose levels;
- smoking tobacco;
- harmful consumption of alcohol;
- poor nutrition;
- low physical activity;
- overweight or obesity.

An important feature of medical examination is not only early detection chronic non-communicable diseases and risk factors for their development, but also conducting brief preventive counseling for all citizens with these risk factors, as well as for individuals with high and very high total cardiovascular risk, individual in-depth and group (patient school) preventive counseling. Such active preventive interventions can quickly and significantly reduce the likelihood of developing dangerous chronic non-infectious diseases in each individual person, and in people already suffering from such diseases, significantly reduce the severity of the disease and the incidence of complications.

Where and when can you get a medical examination?

Citizens undergo medical examination in medical organization at the place of residence, work, study or choice of the citizen, where they receive primary health care (in a clinic, in a center (department) of general medical practice(family medicine), in a doctor’s clinic, medical unit, etc.). Your local doctor (paramedic) or local nurse or a receptionist will tell you in detail where, when and how you can undergo medical examination, and agree with you on the approximate date (period) of undergoing medical examination.

How long does it take to undergo a medical examination?

The examination of the first stage of medical examination, as a rule, requires two visits. The first visit takes approximately 3 to 6 hours (the scope of the examination varies significantly depending on your age). The second visit is usually carried out 1-6 days later (depending on the length of time required to obtain research results) to the local doctor for a final examination and summing up the results of the medical examination.

If, based on the results of the first stage of medical examination, you are suspected of having a chronic non-infectious disease or a high and very high total cardiovascular risk, the local doctor informs you about this and refers you to the second stage of medical examination, the duration of which depends on the amount of additional examination you need.

How to get a medical examination for a working person

According to Article 24 Federal Law of the Russian Federation dated November 21, 2011 No. 323-FZ “On the fundamentals of protecting the health of citizens in the Russian Federation”, employers are obliged to provide conditions for employees to undergo medical examinations and medical examination, as well as freely release workers to undergo them.

What preparation is needed to undergo medical examination:

— To undergo the first stage of clinical examination, it is advisable to come to a medical organization (clinic) in the morning, on an empty stomach, before performing any physical activity, including morning physical exercises.
— Take with you a morning portion of urine in a volume of 100-150 ml. Before collecting urine, be sure to thoroughly clean the genitals. To collect urine and feces, it is preferable to use industrially produced special containers (small containers) for biosamples, which can be purchased at a pharmacy. To perform a urine test, you need to collect a medium portion of urine (start urinating, and then after 2-3 seconds insert a container for collecting the test). Considering the fact that some foods (beets, carrots) can color urine, they should not be consumed within 24 hours before collecting the material. Also, citizens who take diuretics should, if possible, stop taking them, since these drugs change specific gravity, acidity and amount of urine excreted. A relative limitation is the menstrual period in women. It is advisable that the urine sample be submitted to the laboratory within 1.5 hours after its collection. Transportation of urine should be carried out only at above-zero temperatures, otherwise precipitated salts can be interpreted as a manifestation renal pathology, or completely complicate the research process. In this case, the analysis will have to be repeated.
— Persons aged 45 years and older for stool examination occult blood necessary to avoid false positive results for 3 days before medical examination, do not eat meat foods, as well as other products that contain a significant amount of iron (apples, green onions, sweet bell peppers, white beans, spinach), as well as vegetables containing many enzymes such as catalase and peroxidase (cucumbers, horseradish, cauliflower), avoid taking iron-containing medicines, including hematogen, stop taking ascorbic acid, acetylsalicylic acid(aspirin) and other non-steroidal anti-inflammatory drugs (such as voltaren, diclofenac, etc.), stop using any laxatives and enemas. When performing a stool analysis using the immunochemical method, there are no restrictions on food intake (check the research method used with your local nurse or in office 53 medical prevention). Avoid excessively diluting the stool sample with water from the toilet bowl. This may cause incorrect results.
— You must place a sticker with your last name and initials on the container with urine and feces.
— Women need to remember that smears from the cervix are not taken during menstruation, when carrying out any treatment for infectious and inflammatory diseases of the pelvic organs, which in order to reduce the likelihood of getting false results smear analysis, it is necessary to exclude sexual contacts for 2 days before medical examination, to discontinue any vaginal medications, spermicides, tampons and douching.
— Men over the age of 50 must remember that it is better to refrain from undergoing medical examination for 7-10 days after any mechanical impact on the prostate gland (rectal examination, prostate massage, enemas, riding a horse or bicycle, sexual intercourse, treatment rectal suppositories etc.) since they can distort the result of a study of prostate-specific antigen in the blood (tumor marker of cancer prostate gland).
— If you took part in the current or previous year medical research take documents confirming this and show them to medical workers before starting the medical examination.
— The scope of preparation for passing the second stage of clinical examination will be explained to you by your local doctor (paramedic).

What tests and studies does clinical examination involve?

It all depends on age and general health. It may turn out that you are quite healthy man, presenter healthy image life. During the initial examination and questionnaire, the therapist will evaluate your general state and will either refer you for further examination or send you home.

In general, clinical examination includes the following tests and examinations:

  • Measuring height, weight, calculating body mass index.
  • Blood pressure measurement.
  • General or biochemical blood test (as directed).

  • Examination and consultation with a therapist, recommendations.

Everything goes as usual

Below we will tell you how to undergo medical examination at the clinic at your place of residence, how long it takes, and how the procedures are carried out.

On the first day of your visit to the therapist, you are given a questionnaire with 45 questions. Try to answer them honestly, this way you will help the doctor draw up the right plan examinations. Then the therapist conducts an initial examination, measures weight, height, blood pressure and issues a referral for fluorography and general tests blood and urine. If you have recently had an x-ray of your lungs (permissible 1-2 times a year), then this item can be omitted.

As a rule, for those who undergo medical examination, examinations and tests are organized without queues. Therefore, these procedures will not take much time. Women are examined by a gynecologist, men by a paramedic. Once you have completed these basic procedures, you can go home.

After a few days, the test results are ready, you should visit your therapist again, who will make a conclusion and give recommendations regarding disease prevention.

If suddenly there are any disturbances in the functioning of the organs, then additional examinations will be required. That's all. Now you can be confident in your health. Two or three days are worth our peace of mind, right?

  5. During menstruation, you should not take a urine test or take smears.
  6. Before visiting a gynecologist or urologist, you should abstain from sexual activity for several days.

But only half of the population uses it. The rest are sure that all this is for show, and waiting in queues will be tormented. We tell you how to get the most out of the examination and where to save time.

OLGA MOKSHINA

Clinical examination - preventive examination in state clinic. She has two goals:

Determine what diseases may develop in the future due to hereditary predisposition and poor lifestyle;

Reveal dangerous diseases at an early stage.

Here are five steps for those who want to check their health but don't know where to start.

1. Find out when

On one's own. All residents of Russia from 18 to 39 years old who are insured in the compulsory medical insurance system can undergo medical examination once every three years. From the age of 40, in accordance with the new order, it can be taken annually. You go for a medical examination the year you turn 21, 24, 27, 30, 33, 36, 39, and then every year thereafter. Disabled people and WWII veterans, regardless of age, undergo medical examination once a year.

Advice

To avoid confusion, the Internet advises dividing your age by three: if it turns out without a trace, you can go. This is not entirely correct. If you have a birthday at the end of the year, it’s so easy to miss your medical examination. You need to look at the year of birth, not at full age. For example:

I can undergo medical examination from January 1 to December 31, 2017. No need to wait for your birthday.

From the doctor. It happens that clinics call patients and invite them to a medical examination. This happened to me. This is because, according to the law, the local therapist is responsible for medical examination. And he is obliged to ensure that all patients assigned to his site are examined.

From an insurance company. Sometimes you can find out that it’s time to go to the clinic from a thoughtful SMS from the insurance company servicing your compulsory medical insurance policy.

If you are not “age-appropriate” for medical examination, there is an alternative - a preventive examination. It includes fewer procedures and can be done once every two years.

2. Call the clinic

Officially, the order of the medical examination is regulated by the order of the Russian Ministry of Health and the recommendations of the State Research Center for Preventive Medicine. In fact, they do not have clear requirements, and the head physician decides on the spot how exactly to carry out the medical examination. In order not to waste extra time and nerves at the clinic, check in advance on the website or by phone:

Do I need to sign up for a medical examination?

In which line to wait for reception - general or special;

Is it possible to undergo medical examination on weekends or on evening time(this is provided for by the Decree of the Government of the Russian Federation of April 12, 2019);

Where to get directions;

How to prepare for tests.

On the website of Moscow Polyclinic No. 9 they are proactive and indicate all important information. But it's not like that everywhere

If the order does not suit you, you can change the clinic. You have the right to do this once a year.

3. Collect documents

- Passport.

- Compulsory medical insurance policy.

- Questionnaire. Questions about health status, bad habits, diseases suffered by relatives. It is given out at the clinic.

Advice

If you want to spend less time in the clinic or need to think about the questions, download the application form and fill it out at home.

Answer honestly, otherwise doctors will not be able to help you, and the examination will become meaningless

- Direction. In city clinics, referrals are usually received from a local doctor or a prevention clinic. In villages where there is no doctor, see a paramedic. Accurate information should be on the clinic’s website and at the reception.

- Informed voluntary consent for medical intervention. It is issued on site before the examination begins. By law, you have the right to refuse some or all screenings.

In the consent you will be asked to indicate your full name, registration address and number mobile phone

4. Walk through the offices

Clinical examination is carried out in two stages. At the first stage, you will fill out the questionnaire (or proudly hand it in), undergo the required examinations and consult with a doctor. You will be referred to the second one if the doctor suspects some kind of disease and needs to clarify the diagnosis.

But the number of stages is not always equal to the number of visits. Some clinics conduct medical examinations in two visits. For the first time, the patient signs a consent form, fills out a questionnaire, takes tests and undergoes examinations. The second time he is seen by a doctor. But it also happens differently: I went to the clinic three times.

40 minutes

The first time I sat in line for 20 minutes in the prevention room. In her 20s, she signed a consent to undergo a medical examination, filled out a questionnaire, and received referrals for examination. They measured my height, weight, arterial pressure, body mass index was calculated.

180 minutes

The second time I gave urine and blood, went for an electrocardiogram - all together it took an hour. I sat in line to see the gynecologist for almost two hours. The appointment took 10–15 minutes.

20 minutes

The third time I sat in line for 10 minutes to see the therapist, and the same amount at the doctor’s. I was not prescribed any additional examinations. In total, I spent four hours of pure time on medical examination.

Analyzes and examinations

Everyone is measured:

Height, weight, waist circumference;

Arterial pressure;

Body mass index;

Intraocular pressure (once, at the first adult medical examination);

Total cardiovascular risk - probability severe complications and death due to heart and vascular diseases in the next ten years.

Everyone is given directions:

For fluorography of the lungs;

A blood test for sugar and, up to 85 years of age, for cholesterol;

ECG (at the first medical examination in an adult’s life once, from the age of 35 - at each medical examination).

Since 2018, clinical examination no longer includes a general urine test, as well as clinical, detailed clinical and biochemical tests blood.

In addition, all studies and tests available for certain indications from a doctor are listed in paragraph 18 of the order of the Ministry of Health.

Write your year of birth in the box

If you have done any research during last year, directions may not be given. This usually happens with fluorography, which in some clinics is mandatory once a year. The decision is made by the health worker when he writes the referral.

Example

I am 30 years old. During the medical examination, they measured my height, weight, waist circumference, blood pressure, and calculated my body mass index. I've passed urine for general analysis, blood - for sugar, cholesterol and clinical trial , was examined by a gynecologist. I was charged electrocardiogram, since I came to a medical examination for the first time in my life. They didn’t give me a referral for fluorography - I had it at the clinic less than a year ago.

Since 2018, general urine and blood tests have been canceled.
ECG is prescribed for patients over 35 years of age

Therapist

At the end of the first stage, the doctor will report the test results and tell you which health group you are assigned to.

First health group. You do not have chronic diseases and there are few risk factors for their development in the future. In this case, the doctor will limit himself to a brief preventive consultation of no more than ten minutes. The purpose of the consultation is to inform the patient about how to change his lifestyle. For example, a doctor will tell a sweet tooth about the basics of proper nutrition.

Second health group. You do not have chronic diseases, but there is a high probability that they will appear in the near future. The doctor will conduct an extended preventive consultation. Duration - up to 45 minutes. The purpose of the consultation is to motivate a person to change their lifestyle. For example, a doctor will tell a smoking patient about the advantages of quitting cigarettes, give a reminder, and suggest a way to quit smoking.

Third health group. You are at high risk of developing cardiovascular diseases or have chronic illnesses. The therapist will conduct an extensive preventive consultation and tell you which doctor you need to see and how often.

If the doctor suspects any disease, you will be given a referral for additional examinations. After them, you will return to the therapist again.

5. Resolve difficulties

The clinic does not give directions for some examinations

Why. The clinic does not have a license for some types of medical care, the necessary equipment has broken down or the doctor has quit.

What to do. You must be given a referral to another clinic where you can undergo medical examination. If they don’t, write a complaint to the chief doctor. In the document, state all the circumstances and demand either a referral for research or a written refusal.

Make the complaint in two copies. At the chief physician's office, one copy will be returned to you with a signature, seal and reference number, and the second copy will be kept for yourself. That's it, now chief physician must respond to you by mail within 30 days. In practice, they respond earlier, since the regional Ministry of Health obliges chief doctors to promptly resolve conflicts with patients. Most likely, they will call you and ask you to come for a referral.

If a written refusal was received from the chief physician or he did not respond, contact the regional Ministry of Health. The complaint can be sent electronically through the ministry’s website, by Russian Post with a notification, or brought and registered in person. In any case, you are required to respond by mail within 30 days.

The link opens a Google document. To save the sample to your computer, select File → Download as → in the status bar Microsoft Word. Insert your own data instead of the template ones. If it is customary for your organization to write all statements by hand, rewrite them. If the management is satisfied with the version typed on the computer, print it out. You can check with the HR department. In both cases, sign the application

The doctor was formal and did not tell you about your state of health

Why. The doctor wants to submit a report and forget about medical examination until next year.

What to do. Talk to your doctor first. Perhaps he did his best, but you misunderstood each other. If he brushes you off again, write a complaint to the chief doctor. The procedure is the same as in the case of direction.
