12.9. Analysis of the structure of appeal by nosological forms (indicate the number and percentage of diseases in the analyzed group):

Diseases of the circulatory system; - injuries;

Respiratory diseases; - poisoning;

Diseases of the digestive system; - diseases of the nervous system;

Diseases genitourinary system; - diseases of the musculoskeletal system;

Infectious diseases; - skin diseases and subcutaneous tissue and etc.

Note: the work report must be supplemented with tables, diagrams and graphs; conclusions should be drawn, the content of which must strictly correspond to the data presented in the report of the certified specialist.


  1. General conclusion about the work done by the certified specialist over the year.

  2. Conclusion (contains the results of the work done, reflects problems and planned ways to solve them, prospects for further improvement of one’s work, tasks for the future).

  3. Provide a list of literature that the certified specialist uses in the process of work (indicate the names of the authors, the name of the publication and publishing house, the year of publication).

  4. At the end of the work, indicate the position, full name and signature of the medical worker who prepared the certification work.

  5. Review of the submitted certification work (certified by the reviewer - the head of the substation where the certified specialist works).

1. Title page of the certification work.

2. Application from a specialist addressed to the chairman of the certification commission of the Ministry of Health of the Krasnoyarsk Territory (the application indicates the category for which the specialist is applying, the presence or absence of a previously assigned category, the specialist’s personal signature and date).

3. Petition from the chief physician on the institution’s letterhead.

4. Qualification sheet established by order of the Ministry of Health and social development RF No. 808N sample.

5. Minutes of the meeting of the certification commission.

6. Photocopies of the following documents:

Work book (last entry “Currently working”);

Diploma of completion of an educational institution;

Documents on advanced training in the certified specialty;

Specialist Certificate;

Certificate of existing qualification category in the certified specialty;

Marriage certificate (if the surname was changed after receiving a diploma, certificates);

A copy of the order of the head of the institution to extend the validity of the qualification category (for specialists who have returned from maternity leave or childcare leave up to 3 years).

All photocopies of the documents listed in paragraph 5 must be certified by a specialist from the HR department.

7. Report on the specialist’s work for the year (according to the diagram).

8. Review of the certification work.
Certification documents are filed in a plastic folder with a transparent top in the above sequence and are located in chronological order, are not included in multifors (transparent files).

Certification work must be in Word format, Times New Roman font, size 14 pt, single line spacing, page parameters (margins): top - 2 cm, bottom - 2 cm, left - 3 cm, right - 1.5 cm, headings are highlighted in bold type, the size of headings does not exceed 14 pt.
