    Good for inflammation hip joint. I put only 3 suppositories, one at night, then switched to Mydocalm and Elbon Ultra, the pain was no longer so severe. As a result, no side effects and a good result.

    I suffered from mengito encephalitis in 2002.. over the years my knees become sad and my veins sag, I get tired, I drank calcium, but to no avail.

    I don't know how to write correctly! but I started injecting Dikloberl No. 75 and after it my leg is very tight! that after the injection I almost can’t stand on it! Do you think this is normal or should I stop injecting myself with it?

Application and dosage

Orally, without chewing, during or after meals, adults - 25-50 mg 2-3 times a day. Upon reaching the optimal therapeutic effect the dose is gradually reduced and switched to maintenance treatment at a dose of 50 mg/day. The maximum daily dose is 150 mg.

For children over 6 years of age and adolescents, only tablets are used normal duration actions 25 mg; daily dose – 2 mg/kg child.

For juvenile rheumatoid arthritis, the daily dose can be increased to 3 mg/kg. The tablets should be taken whole, without chewing, on an empty stomach, with a small amount of liquid.

Long-acting diclofenac is prescribed 100 mg once a day (for dysmenorrhea and migraine attacks - up to 200 mg/day). When taking 100 mg retard, if it is necessary to increase the daily dose to 150 mg, you can additionally take 1 regular tablet (50 mg).

IV drip. The maximum daily dose is 150 mg. Immediately before intravenous administration, diclofenac (contents of 1 ampoule - 75 mg) should be diluted in 100-500 ml of 0.9% NaCl solution or 5% dextrose solution (after adding sodium bicarbonate solution to the infusion solutions - 0.5 ml 8.4% or 1 ml 4.2% solution). Depending on the severity of the pain syndrome, the infusion is carried out for 30-180 minutes.

In order to prevent postoperative pain, an infusion is carried out with a “shock” dose of the drug (25-50 mg over 15-60 minutes). Subsequently, the infusion is continued at a rate of 5 mg/hour (until the maximum daily dose of 150 mg is reached).

For treatment acute conditions or to relieve an exacerbation of a chronic disease, intramuscular (single) administration is also possible; further treatment is continued with oral diclofenac, taking into account the maximum daily dose of 150 mg (including on the day of injection). Injection solution injected deep intramuscularly for no more than 2 weeks.

Rectally, 50 mg 2 times a day or 100 mg 1 time a day. Migraine attack - rectal suppositories at a dose of 100 mg at the first sign of an attack. If necessary, repeat 100 mg.

special instructions

In order to quickly achieve the desired therapeutic effect, take 30 minutes before meals. In other cases, take before, during or after meals, unchewed, with a sufficient amount of water.

Because of important role Pg in maintaining renal blood flow, special care should be taken when prescribing to patients with heart or renal failure, as well as when treating elderly patients taking diuretics, and patients who, for any reason, have a decrease in blood volume (for example, after a major surgical intervention). If diclofenac is prescribed in such cases, monitoring of renal function is recommended as a precaution.

In patients with renal failure with CC less than 10 ml/min, Css of metabolites in plasma should theoretically be significantly higher than in patients with normal function kidneys, but this is not really observed, since in this situation the excretion of metabolites with bile increases.

In patients with liver failure ( chronic hepatitis, compensated cirrhosis of the liver) kinetics and metabolism do not differ from similar processes in patients with normal liver function. When carrying out long-term therapy, it is necessary to monitor liver function, peripheral blood patterns, and stool analysis for occult blood.

During the treatment period, the speed of mental and motor reactions may decrease, so it is necessary to refrain from driving vehicles and other potentially potentially dangerous species activities that require increased concentration and speed of psychomotor reactions.

Interaction

Increases plasma concentrations of digoxin, methotrexate, Li+ drugs and cyclosporine.

Reduces the effect of diuretics; against the background of potassium-sparing diuretics, the risk of developing hyperkalemia increases; against the background of anticoagulants, antiplatelet and thrombolytic drugs (alteplase, streptokinase, urokinase), the risk of bleeding increases (usually the gastrointestinal tract).

Reduces the effect of antihypertensive and hypnotic drugs.

Increases the likelihood of side effects of other NSAIDs and corticosteroids (bleeding in the gastrointestinal tract), the toxicity of methotrexate and the nephrotoxicity of cyclosporine.

ASA reduces the concentration of diclofenac in the blood. Concomitant use with paracetamol increases the risk of developing nephrotoxic effects of diclofenac.

Reduces the effect of hypoglycemic drugs.

Cefamandole, cefoperazone, cefotetan, valproic acid and plicamycin increase the incidence of hypoprothrombinemia.

Cyclosporine and Au preparations increase the effect of diclofenac on Pg synthesis in the kidneys, which is manifested by increased nephrotoxicity.

Simultaneous administration with ethanol, colchicine, corticotropin and St. John's wort preparations increases the risk of bleeding in the gastrointestinal tract.

Drugs that cause photosensitivity increase the sensitizing effect of diclofenac to UV irradiation.

Drugs that block tubular secretion increase the plasma concentration of diclofenac, thereby increasing its effectiveness and toxicity.

The inflammatory process caused by the active activity of pathogenic microorganisms can provoke many diseases. Eliminate painful sensations and relieve swelling of soft tissues, drugs from the NSAID group (non-steroidal anti-inflammatory drugs), one of which is Dicloberl, help. The main component diclofenac sodium helps in the treatment of many diseases accompanied by inflammation. What are its features and advantages, as well as in what cases it is best to use it, we will analyze further.

pharmachologic effect

Dicloberl is a non-steroidal anti-inflammatory drug based on phenylacetic acid. Capable of providing the following factors effects on the body:

  • relieves inflammation in soft tissues;
  • has an antipyretic effect;
  • helps reduce swelling;
  • reduces the sensitivity of the nerve fibers of the affected area.

All this is achieved by inhibiting the synthesis of proglandins, which signal the presence of foreign bodies in the body.

When taken orally his drug maximum concentration observed in plasma in 1-15 hours. In this case, diclofenac is completely absorbed in the intestine and excreted in the urine 15-20 hours after administration. Parenteral (intramuscular) introduction allows speed up the absorption process, however efficiency is much lower than when taken orally. Usage rectal suppositories increases the percentage of absorption, allowing you to achieve maximum concentration in blood plasma at the maximum short time .

Dicloberl undergoes a partial course of metabolism, binding well to blood proteins. Inactive metabolites, formed as a result of conjugation by hepatocytes, are excreted by the kidneys. The rest of the metabolites are excreted by the intestines within 12-15 hours.

  • chronic otitis in the acute stage;
  • pharyngotonsillitis.

As a prophylactic agent, Dicloberl widely used before surgery on ENT organs (removal of polyps, tonsils, adenoids), as well as in gynecology.

Reception of Dicloberl as the main one medicinal product is possible only after consultation and prescription of a doctor. Fever or joint pain is not a key factor affecting the cost-effectiveness of using this medication. Only after comprehensive examination, based on the data obtained on the state of health and diagnosis, the drug is able to have a beneficial effect on the body.

Composition and release form

The main active ingredient of the drug is sodium. Its concentration varies depending on the form of release, of which there are several:

  1. Rectal suppositories– may contain 100 and 50 mg of diclofenac in one suppository. In addition, there are also auxiliary components:
  1. Ampoules for intramuscular injections – contain diclofenac sodium 75 mg in each ampoule. Auxiliary components:
  • propylene glycol;
  • injection;
  • acetylcysteine;
  • benzyl alcohol;
  • sodium hydroxide.
  1. Pills contain 50 mg diclofenac sodium in each capsule. Related ingredients that improve the transportation of the tablet, as well as its absorption in the gastrointestinal tract:
  • magnesium stearate;
  • corn starch;
  • yellow pigment (dye E211);
  • simethicone emulsion;
  • talc;
  • glucose monohydrate;
  • titanium dioxide;
  • methacrylic acid.
  1. Dikloberl-Retard- these are tablets, the action of which allows the accumulation of the active element in the body, which explains its prolonged effect on the source of inflammation. It is used for protracted inflammatory processes that have a chronic course. One tablet contains an increased concentration of diclofenac - 100 mg. Inactive components are also present:
  • sucrose;
  • maltose monohydrate;
  • corn starch;
  • gelatin;
  • white dye;
  • talc.

The feasibility of using one or another form of release directly depends on indicators such as:

  • type of disease;
  • duration and characteristics of the course of the disease;
  • individual sensitivity to components;
  • presence of chronic diseases.

Only a doctor is able to choose the correct form of Dicloberl and its dosage, knowing all the details and nuances about the course of the disease and its characteristics.

Method of administration and duration of treatment

For parenteral use Dicloberl is injected deep into the gluteal muscle. Daily dosage amounts to 75 mg of substance– this is 1 ampoule. In rare cases, daily permissible norm can be doubled (150 mg). Duration the course of treatment should not exceed 10 days. If it is necessary to continue treatment, it is carried out using tablets and rectal suppositories.

Daily dose Dicloberla in tablets amounts to 50 mg. Sometimes it is increased to 150 mg, dividing the dose into 2-3 doses. Tablets are taken directly with meals(this reduces the risk of irritation of the stomach walls) with a small amount of warm water. The duration of the course depends on the indications and doctor's prescription.

Rectal suppositories Dicloberl is injected directly into the rectum after it has been cleared of feces. If you have constipation, you can give a cleansing enema, which will help gently remove stool and facilitate the absorption of diclofenac. The daily dose should not exceed 150 mg. For better absorption and digestibility by the body, it is divided into 2 doses: morning and evening. The duration of treatment should not be more than 5 days. When a suppository is inserted into a full intestine, the effectiveness of treatment decreases sharply.

Dicloberl-Retard capsules are used once a day, since an initially increased concentration of diclofenac (100 mg) has more successful treatment, but can provoke adverse reactions in case of overdose.

Contraindications

Diclober is not used in the treatment of inflammatory processes when the following contraindications exist:

    • peptic ulcer and duodenum(especially in chronic cases during the period of exacerbation);
    • gastritis and small stomach bleeding;
    • disorders of hematopoietic functions;
    • bronchial asthma and others chronic diseases lungs;

Various release forms and dosages easily make it possible to choose the most convenient dosage regimen suitable for each specific case. Injections and suppositories are most often used in gynecology. While tablets and capsules are convenient for long-term treatment

Properties

Dicloberl is described as a non-steroidal anti-inflammatory drug. It is a derivative of phenylacetic acid.

Able to work as an active antipyretic agent, providing a timely anti-inflammatory effect.

Well resists inflammatory processes in soft tissues.

It is able to penetrate into various stages of the development of inflammatory processes and acts on them in a targeted manner, preventing the source of inflammation from spreading further.

If inflammatory processes occur after surgery or as a result of injuries, timely administration of Dicloberl will help not only eliminate the pain effect when moving, but will also prevent the formation of surgical swelling.

In case of damage to soft tissues, it reduces sensitivity and helps to minimize the susceptibility of nerve fibers in the damaged area.

Penetrating into the body, Dicloberl begins to actively work to inhibit the proliferation of prostaglandins, which are responsible for the reproduction of foreign bodies in the body. Thanks to this, it is easier for active substances to accumulate their strength in a timely manner to fight the source of inflammation.

Dicloberl is also capable of providing a mild analgesic effect even if the source of pain is not rheumatic in nature.

When consuming pills, the maximum possible concentration in the body occurs no earlier than an hour later and no later than 15 hours. It is metabolized without residue and excreted in the urine no less than 20 hours after the procedure.

After injections, the effectiveness of the drug is slightly lower than that of tablets. But at the same time, the absorption rate is faster, the maximum concentration occurs after 10-20 minutes. Penetrates into the joint fluid without problems. The process of releasing active substances from the body also occurs more actively. 12 hours is enough for the body to get rid of all the products of the drug.

When administering suppositories, the maximum permissible concentration occurs within half an hour.

Penetrating into the body, approximately a third of the total volume is metabolized. Once in the intestines, it is eliminated and excreted in the feces. The rest is excreted by the kidneys.

Composition and release form

The main component of Dikloberl is diclofenac sodium. To make it easier to use in treatment, Dicloberl is produced in three varieties. In the form of suppositories, solution for injections and dragees. Each sample is made in its own concentration. Such a variety of forms allows you to choose the most suitable treatment regimen for a given case.

Dikloberl in suppositories is placed only rectally. Available in concentrations of 100 and 50 mg. In addition to the main component, corn starch is added to their composition, ethanol, solid fat, propyl gallate.

The injection solution is already ready for use and does not require additional dilution. Each ampoule contains 75 mg sodium. As binding ingredients it contains propylene glycol, injection solution, acetylcesteine, benzyl alcohol, sodium hydroxide.

Each Dikloberl tablet contains 50 mg of diclofenac sodium. As accompanying components, it contains magnesium stearate, corn starch, yellow food coloring, simethicone emulsion, talc, glucose monohydrate, titanium dioxide, methacrylic acid.

Dikloberl dragees may also include 100 mg of diclofenac sodium. This dosage has its own name, Dicloberl Retard capsules. Dikloberl Retard as accompanying ingredients includes sucrose, maltose monohydrate, the same corn starch, gelatin, white food coloring and talc. All this allows the pill to better dissolve and be absorbed in the body. Diclofenac Retard, unlike its lightweight companion, is capable of accumulating diclofenac sodium in the body. This is why the results last longer after taking it.

Only the doctor can decide which form of the drug to choose for treatment.

The choice can be influenced not only by age and available accompanying illnesses, but also the body’s susceptibility to the components of the drug, as well as the duration and severity of the disease.

Indications for use

It’s not for nothing that this drug has so many design options. active substance. With a clear understanding of the diagnosis, it is easy to choose the most appropriate form in a particular situation.

Capsules

Dicloberl capsules are often prescribed for injuries various kinds(sprains, fractures, dislocations), in such conditions the drug can cope well with both painful sensation, and prevent possible inflammation. Dentists often recommend taking a course of capsules after minor dental and orthopedic interventions (tooth extraction). The purpose of the prescription is to relieve pain and prevent inflammation. A course of capsules can also minimize inflammatory processes and relieve pain in various diseases of the musculoskeletal and nervous system spinal column.

Dicloberl in tablets works well with ENT pathologies accompanied by pain and inflammation; in combination with other drugs, it can cope with especially complex forms of these diseases (otitis in acute form, pharyngitis, tonsillitis). Included in complex therapy with systemic joint disease, especially when it is chronic (Bechterew's disease).

Tablets are also prescribed after dental surgery. In gynecology, Dicloberl is often prescribed for inflammatory pathologies, especially if the disease is accompanied by pain. It treats gout well in the acute stage, accelerates the recovery process after injury, especially if there is an inflammatory process in the body. Dicloberl is often included in a complex of drugs for the restoration of spinal lesions for its ability to relieve pain well. Helps tidy up soft fabrics after damage by rheumatism.

Dicloberl suppositories are often prescribed for systemic connective tissue disease ( rheumatoid arthritis, including its juvenile form), with systemic joint disease, especially when it is chronic (Bechterew's disease), with spondyloarthritis.

Instructions for use

Dicloberl in ampoules

It should be injected deeply into the gluteal muscle, slowly. One ampoule is equal to the daily requirement. If the situation requires it, it can be increased, but not more than twice. In this case, it is better to take a break of several hours between injections.

Most often, injections are given only in the first few days after the onset of symptoms of the disease, in order to quickly and effectively relieve symptoms. As a last resort, injections continue to be given, but not more than 10 pieces. Further treatment produced by using another form, such as candles.

The daily intake of Dicloberl should not exceed 150 mg. The optimal daily portion is considered to be 50 mg. If it requires an increase, then the tablets must be taken in several approaches. You need to take the pill during meals, not forgetting to wash it down with a small portion of warm water. As soon as the state will enter normal, daily intake should be reduced to 50 mg.

In special cases, children over 6 years of age are also prescribed Dicloberl in tablets at a daily dose of 25 mg. You should know that the daily dose should not exceed 2 mg/kg. This formula is used as a starting point when building a treatment regimen.

Dicloberl in capsules

One capsule of Dicloberl Retard per day is enough for successful treatment severe pain. In rare cases, this rate is doubled, bringing it to 200 mg. After the primary effect is achieved, the rate can be gradually reduced. First, up to 150 mg, drinking a capsule and a tablet per day, and then up to one capsule per day. It must be taken into account that such a large dosage, although it is much more successful in combating the disease, is much more more likely to appear side effects. Therefore, this dosage is taken only under the close supervision of the attending physician.

The duration of treatment is adjusted by the doctor.

Dikloberl candles

Dikloberl suppositories are placed in the rectum. If the patient has constipation, it is better to prepare the intestines for the procedure in advance by forcibly emptying the bowels. If these procedures are not carried out, the effect of treatment will be reduced. Typically the daily intake ranges from 50-150 mg. The suppositories need to be administered over several procedures. Subsequently, the consumption rate is reduced to a candle per day. As a rule, treatment lasts no longer than five days.

Overdose

If the instructions attached above are not followed and daily norm, an overdose may occur. In what form does it appear?

First of all, dizziness may appear, even fainting. There may be jumps in blood pressure and increased heart rate. Nausea and vomiting accompanied by diarrhea are possible. Cramps of the limbs, rashes on the skin in the form of red spots, pain in the liver and kidneys is possible. Stomach bleeding may occur.

At the first signs of an overdose, you should rinse your stomach and induce vomiting yourself. If a severe allergic reaction occurs, take a double dose of the antihistamine and call an ambulance.

Side effects

At different situations and depending on the “bouquet” of diseases, the patient may experience adverse reactions:

  • Pressure deviations may occur. Sharp jumps are likely blood pressure upward. There may be pain in the chest area, and tachycardia may also occur.
  • There may be a decrease in hemoglobin in the blood. The white blood cell count may also drop, against this background the body’s susceptibility to bacterial and fungal diseases increases.
  • Nausea, vomiting, and the development of gastric ulcers are just some of the disorders that are noted in the gastrointestinal tract.
  • Possible development depressive state, excitability, headaches.

To minimize such reactions, you need to go through as much full examination before procedures. This will avoid possible problems and will significantly reduce the likely risk of treatment.

It is better not to take risks and not to include other non-steroidal anti-inflammatory drugs in parallel use with Dicloberl. This combination is fraught with subsequent stomach ulcers.

Phenytoin, Rifampicin - they speed up the metabolic process and can reduce the effect of treatment.

Diclofenac can almost halve the result of treatment with Kolestyramine and Colestipol.

If one of the drugs cannot be excluded, you should take a break between procedures of a couple of hours.

Prescribe Diclofenac with caution and when diabetes mellitus. There are known situations where, due to taking Dicloberl, the glucose level in the blood changed sharply. Moreover, the readings can increase sharply and, on the contrary, fall below normal.

It is also better to exclude Sulfinpyrazone and Probenecid during treatment with Dicloberl due to their ability to retain diclofenac sodium.

You should also avoid taking medications that weaken the ability of blood to clot and inhibit the formation of blood clots. Since Dicloberl enhances the effect of such drugs.

A parallel course with drugs responsible for lowering blood pressure and diuretics can reduce the effectiveness of taking them.

Such combinations should be prescribed extremely carefully, especially among older people. Be extremely careful to ensure that the patient drinks enough water.
